Summer 2025 Course Completer Form
| This is a mandatory form for students who took a course over the summer outside of our district, and wish to use the course as a prerequisite. Please note that you must submit documentation showing your FINAL GRADE in the course (progress reports will not suffice). If you took a math course and have not yet submitted an MHS math waiver form, you must complete that form FIRST before submitting this one. Email mhsmath@smuhsd.org |
